Office Hours

All office hours are by appointment only, Monday through Friday 9:00am to 4:30pm.


Cancellations

If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice.  This courtesy on your part will make it possible to give your appointment to another patient.


Emergencies

Our office provides on-call service for emergencies, 24 hours-a-day, 7 days-a-week through the Academy of Medicine, Toledo, Ohio. Phone Number (419) 473-3200.


Health Insurance Information

Our organization joins with most health insurance organizations which cover surgical procedures.  Covered procedures will differ with each insurance plan -- our administrative staff will assist you in determining eligibility. Patients are responsible for deductibles, co-payments and remaining balances.
